As a Provider Agency of the Home and Community-Based Services- Adult Mental Health (HCBS-AMH) program, we offer an array of services individualized to the needs of each program participant. We work with each individual to support recovery and ensure a successful reintegration into his or her community of choice.

Mission Statement

Our mission is to provide home and community-based services to adults with a diagnosis of SMI and extended tenure in psychiatric hospital as well as those individuals with frequent arrests or ED to enable them to achieve the highest level of potential in their day-to-day independent activities as well as help them experience successful tenure in their community.
